Please note: change to therapeutic range from 17/11/2025.

Therapeutic range (pre-dose): 10 – 15 mg/L
Target level may vary with the site of infection. Please contact pharmacy.
Post-dose (30 minutes) is not recommended.

There are no targets for vancomycin spot levels. Vancomycin level must be correlated with the time of the last dose. Please review the pharmacy progress notes or contact pharmacy for assistance with interpreting the level.
