Request should specify dose, route, time(s) of dose/infusion and time of sample collection.

Post Dose specimen collection should be 30 mins post completion of administration

Neonates:  It is recommended to take the therapeutic level immediately before the third dose is due. Only after the level is obtained the drug should be given.

Adults :

Pre-dose:   < 0.5 mg/L
Post dose:   consult

Neonates:

Pre-dose < 1 mg/L.
