Request:

APS request form signed by the Oncologist or Oncology Registrar is mandatory

Authorisation:

Pathologist authorisation by Prof Catriona McLean or delegate is required prior to tests being performed

Pathologist block selection of tissue is required

Registration:

Test registration into PathNet is performed by Anatomical Pathology

Register with a new BX accession (required for billing)

A pathology request form completed and signed by an Oncologist or Oncology Registrar is mandatory

All solid tumour molecular test requests must be discussed directly with Prof Catriona McLean (ph 907 63153) prior to testing being performed
